To implement the FIRST STAGE follow these four simple steps:

1. Visit www.howsyourhealth.com and choose to complete a sample quiz. Make sure that the person who would oversee this program "test-drives" it. Finally, invite the office staff to try it.

2. Please note that you can change the five questions appearing at the end of each quiz, include an open-ended question, and create up to ten subgroups to compare responses. You may change parts of the "letter" and "action form" that appear at the end of the quiz. None of these changes require surrender of user confidentiality. Based on your feelings and staff input, plan to customize it to your specifications. You can always change it later.

3. Now order the program. You are entitled to 2000 uses per order and subgroup orders (e.g. extra physicians) are each entitled to 2000 additional uses. Each subgroup will receive a quarterly report comparing it to the overall group. However, we recommend that you begin without subgroups unless you have had a free consultation with us (see below).

4. Think about how you will enroll users. To invite persons to use www.howsyourhealth.com, you might try posting or distributing the following general invitation

"As a healthcare provider for _____, we are committed to finding new and better ways to improve your health and medical care. We are currently working with a university based internet research program called "How’s Your Health", which can help improve your ability to maintain a healthy lifestyle. Here is how it works.

First, find an internet connection. There is a computer in _____, which you may use for this quiz. Or, you may use any computer, which has internet access, at home, at a friend's home, or at the library. It takes about 5-15 minutes to complete.

When using a computer with an internet browser (Netscape or Internet Explorer) at ____, at home, at a friend's, or at the library, enter http://www.howsyourhealth.com in the address window. Click on the SURVEY option and follow the steps to the "How's Your Health" quiz. When requested, enter the password code _____ and complete the quiz.

You and your family members, can complete the quiz by selecting the appropriate age and sex. Based on your answers, you will be given special information personalized to your needs. All your answers are confidential.

Upon completion, you will receive an "Action Form" which will summarize your quiz results. We encourage you to share this with your doctor at your next scheduled office visit.

If after taking the quiz, you have questions, do not hesitate to talk to____ or feel free to call us at _____.

Thank you for taking the time to complete this important quiz.

Or, you might attach the letter to the back of a poster like this.

If you wish free printed copies of the poster please call 1 800 369 6669.

How to Use the Action Form to Ensure the Best Outcomes of Medical Care

The Assets Section of the Action Form indicates the things that are going well for the person who completed the Improve Your Medical Care Health questionnaire.

The left column of the Needs Section of the Action Form lists those things that may not be going well and may benefit from more information and more discussion. Based on these needs, readings have been suggested. The suggested readings are listed in the small box on the right. Under this box is another that indicates the possible risk category for the person completing the questionnaire and the reasons for the risk category.

Clinicians who receive this Action Form might:

1. Review the list of needs to be sure that they are correct.

2. Pay particular attention to needs that appear in bold italics. This notation indicates a need that the clinician may be unaware of the problem.

3. Annotate corrections and actions for any of the listed needs in the central column of the Action Form.

4. Place the Action Form in the medical record.

THE SECOND STAGE

An organization should use the SECOND STAGE after it has become familiar with the general function of www.howsyourhealth, and received a quarterly report comparing its quiz users to others. An organization that has had fewer than 100 users/quarter should generally not move to the second stage until it has determined how to improve the response rate.

Successful implementation of the SECOND STAGE has three requirements.

1. The organization has developed explicit methods to address these questions:

• how it will respond to surrender of user confidentiality? What safeguards are in place?

• how will it respond to email receipt of a users "Action Form" if this option is chosen? Who is accountable for action and security?

• how will it respond to a user responding to an invitation to register for special care? Who is responsible for action and security?

2. The organization has identified a target for improvement or special care. For example, early in our work we recognized that users who have pain AND emotional concerns were more bothered by their pain than those without emotional concerns. We successfully addressed this by inviting persons with pain and emotional problems to register for nurse follow-up and discussion of the exercises in the readings on pain. On www.howsyourhealth.com you might choose to have the invitation look like this.

AN INVITATION

Did you answer that in the past four weeks that:

• you have had moderate or severe PAIN?

• EMOTIONAL PROBLEMS have bothered you quite a bit or extremely?

• SOCIAL ACTIVITIES have been limited quite a bit or extremely?

If you answered "YES" to all of these three questions, we invite you to register for a special pain management program run by Dottie, our office nurse. She will call you at your convenience and explain the program to you.

If you agree to accept the invitation your personal information and responses will be sent to our office to the attention of Dottie.

3. The organization sets up a method to review STAGE TWO results every quarter.
